Calibration Service Form

A Calibration Service form must be completed and enclosed with each shippment.
For the accuracy of your information we will no longer be accepting handwritten forms. 

How to use the TTE Calibration Service Form

Fill this form out making sure to include your contact information, service level selection and payment method.
Then print out your completed form and include it with your pipettes.
For your convience this form maybe saved to your computer and/or automatically remember your information for your next service.

Please follow these directions to make certain that your pipettes are safe during shipping and help prevent package damage or loss:
1. Use a box strong enough to support the weight of the contents-or our pipettes.com box.
2. Use at least two inches of appropriate cushioning material to protect items from each other and the corners, sides, top, and bottom of the box
3. Reinforce package edges to protect from bending
4. Securely seal package closures and seams with reinforced tape
5. Include complete address information and telephone numbers on the label
6. Insure the instruments for their replacement costs
